Can anyone shed a little light on things for us?

Hi

We are newbies to this process so be gentle!!!

We have recently attended the HARI clinic and have been advised (although it has felt a little more like an order!!) to proceed with Gonadothophin Stimulation because this is a less invasive procedure than IVF.  However the success rates are only about 15% and each procedure requires a large quantity of our limited sperm store.

Can anyone shed some light on this procedure - is it really worth trying or are we better off bypassing this and go straight to IVF where better use could be made of our store with a higher likelihood of success?  We feel like we have been rail roaded a little by HARI without proper explanation on why this is the best option.

Confused in Dublin...

Hi Lucy, If I am understanding you correctly, the physician you consulted with, suggested IUI, standing for intrauterine insemination? If that is the case, it is less invasive than IVF with or without ICSI. For the IVF you will most likely have gonadothrpin stimulation as well, even if it is the so called "natural" cycle! IUI does have a lower success rates, much lower than IVF. It is up to the patient at the end if they will agree to the suggestion from their physician. I am from the states, and the physician we have seen there explained to us, that IUI is more for couples where there is an issue with the cervical mucus, preventing the sperm to get on time, and for couples with a mild male factor infertility.

Not sure what your situation is, but it would be best to get a second opinion.

For the IUI, you will most likely be asked to give yourself, or someone else to give you stimulation injections, until your follicles reach a minimum size, usually this is 18 mm. You will be asked during the stimulation to have some visits to the clinic and the will most likely draw blood to check your hormone levels and have vaginal ultrasound to monitor the follicular growth. Once, your follicles reach the necessary size, most likely you will be given a trigger shot, either pregnyl or ovidrel, which helps the eggs to reach the last phase of maturation before they are released by the follicles. Then, you husband will have to give material the day of the insemination. The sperm will be washed, a special procedure, and will be concentrated, and with a small catheter will be place in your uterus. Fairly painless procedure, excluding the shots.

I can not give you suggestions if it is worth going straight for the IVF! It is up to the individual situation, the lab results, the female hormones, med. history, etc.

And do not worry for the limited sperm store! Sperm is produced and comes to maturation about every 70-90 days, new batch; but that does not mean that the old batch ends at once!!!
